  • Proof of vehicle ownership
  • Proof of Kansas residency
  • Completed Schedule C
  • Proof of Insurance
  • Form 2290 - Heavy Use Tax (if applicable)
  • Updated MCS 150 (if applicable)
  • Lease Agreement (if applicable)
  • Completed Schedule D (IRP only)
  • IFTA Number (IRP only)

Renewing A Fleet

  • Renewal Form - Note any garage address changes, indicate any weight changes and delete any vehicles no longer in the fleet
  • Proof of Insurance
  • Updated MCS 150 (if applicable)
  • Form 2290 - Heavy Use Tax (if applicable)
  • IFTA Miles or Schedule B (IRP only)

Adding A New Vehicle
