Home Visiting Program
Newborn Home Visit (newborns aged 0-2 months)
- For moms and newborns 0-2 months
- One or two home visits with the MCH Home Visitor and/or Registered Nurse
- The infant will receive a nursing assessment including a weight check
- Information about infant care and safety such as safe sleep, childproofing, car seat safety, and early childhood education
- Education about healthy pregnancy, infant development, and mother's wellbeing
- Referrals to additional community resources
Home Visit (children aged 0-5 years old)
- For families with children 0-5 years old interested in a higher level of support
- Developmental screening (identify delays and celebrate milestones)
- Weekly home visits with a Licensed Social Worker or Registered Nurse
- Emphasis on family well being
- Education about pregnancy and early childhood development
- Referrals to additional community resources
