Home Visiting Program

Newborn Home Visit (newborns aged 0-2 months) 

  • For moms and newborns 0-2 months
  • One or two home visits with the MCH Home Visitor and/or Registered Nurse  
  • The infant will receive a nursing assessment including a weight check
  • Information about infant care and safety such as safe sleep, childproofing, car seat safety, and early childhood education 
  • Education about healthy pregnancy, infant development, and mother's wellbeing 
  • Referrals to additional community resources

Home Visit (children aged 0-5 years old)

  • For families with children 0-5 years old interested in a higher level of support
  • Developmental screening (identify delays and celebrate milestones)
  • Weekly home visits with a Licensed Social Worker or Registered Nurse
  • Emphasis on family well being 
  • Education about pregnancy and early childhood development
  • Referrals to additional community resourcesbabies and their parents
